Two New Articles on PC Screening (29th June)

It may take a while for me to fully digest these two articles with my initial opinion being that both articles are of interest and hold some merit but there are comparisons/findings that on first reading are completely groundless!

For Example:

Prostate Cancer: Current Evidence Weighs Against Population Screening

"The real impact and tragedy of prostate cancer screening is the doubling of the lifetime risk of a diagnosis of prostate cancer with little if any decrease in the risk of dying from this disease. In 1985, before PSA screening was available, an American man had an 8.7% lifetime risk of being diagnosed with prostate cancer and a 2.5% lifetime risk of dying from the disease.18 Twenty years later, in 2005, an American man had a 17% lifetime risk of being diagnosed with prostate cancer and a 3% risk of dying from prostate cancer."

The above statement is completely flawed in my opinion as who can possibly know what the lifetime risk of being diagnosed and risk of dying would be after 1985 if there were no screening in place,within the USA!

The 'modern' world where change in lifestyles,food,packaging and antiperspirants for example appear to be just a few factors that are adding to the increase in PC cases.


Think I've re-read enough of the two articles to know they are basically a cut and paste exercise without going deeper into each part they cite along with using emmotive language to push their own view forward,in some cases.

I do agree with some of their conclusions though but not the way they have gone about it!

The new AUA guidelines are just about right,in my opinion.

Next month, European Urology will publish the EAU position statement on screening for prostate cancer which takes into consideration the recent scientific information on randomised screening studies on prostate cancer (Schröder et al, NEJM 2009). The EAU adopts the conclusions of the ERSPC study and recognises the benefit of screening in terms of mortality reduction, as well as the adverse effects of overdiagnosis and overtreatment of prostate cancers which could be quantified for the first time in the setting of a randomised screening study.

My secret battle with cancer,by Strictly Come Dancing judge Len

"Strictly Come Dancing judge Len Goodman has undergone surgery to remove a cancerous tumour in his prostate gland, he has revealed.

He had the operation three weeks ago – without telling any of his BBC colleagues – and now says he feels ‘right as rain’.

The 65-year-old told The Mail on Sunday he will be back on the show in the autumn, and credited his swift recovery to doctors spotting the disease early at his annual check-up."




"Len admitted he feared the worst when the disease was first discovered, but remained upbeat during his treatment.

‘I feel good,’ he said. ‘I was obviously worried at the start. We all worry if we get something like this. You do feel a bit vulnerable, but as long as you come out the other side all right, why worry?’

The private operation at a Bupa hospital in Blackheath, South-East London, was conducted by one of Britain’s leading urologists, Peter Thompson.

‘I was confined to the hospital for four days, and then it’s been a couple of weeks of convalescing and recuperating,’ he said.

‘The trouble with doctors is they won’t tell you definitely what you should do. They leave it to you. So yours is an uneducated guess. They said I could have radiation, but I thought the best thing was to have it removed.

‘It was left to me and I felt that was the best route, but whether it is or not I don’t know. You’re a bit sore for a while and you can’t play golf, which is the biggest blow at the moment. But I will be able to in a month’s time or so – I can’t wait.’

Len, who runs his own dance school in Dartford, Kent, urged middle-aged men to go for regular check-ups, saying: ‘Men, probably more than women, tend to ignore their health.

‘As long as you get checked up every year, then medicine is such now that if you do get something, then hopefully they can catch it early and you have a good chance. ‘I think that’s the most important thing – men of a certain age should have their check-up every year.’

Many UK cancer deaths 'premature' (25th June)

"Up to 15,000 people aged over 75 may be dying unnecessarily from cancer each year in the UK, according to research.

The North West Cancer Intelligence Service said their lives would be prolonged if UK cancer survival rates matched the best in Europe and the US.

Researchers said delays in diagnosing cancer in the UK and poorer treatment after diagnosis may be to blame.

The government's national cancer director said urgent action was needed over the study's findings. "

Kanu becomes Cancer Centre's goodwill ambassador

Looks like the new cancer centre is taking shape as previously reported on:

"THE International Cancer Centre Abuja (ICCA) has named Super Eagle's captain, Kanu Nwankwo, the Centre's Goodwill Ambassador.

At an investiture held yesterday in Abuja, the First Lady and initiator of the Centre, Turai Yar'Adua, said the honour done to Kanu was in recognition of his immense contribution to the less privileged in the society, especially his Heart Foundation.

Represented by the Chairman of the Steering Committee and Federal Capital Territory (FCT) Minister, Senator Muhammed Adamu Aliero, Turai Yar'Adua said Kanu has done the nation proud in various ways, stressing that the Centre would benefit immensely from the support of the likes of Kanu.

He, therefore, charged him to use his enviable position to reach out to many Nigerians, both at home and in the Diaspora, to support the Centre, with a view to reducing the cancer pandemic in the country. Aliero used the occasion to call on all Nigerians to support the initiative, especially the forthcoming launch slated for July 18 in Abuja.

The minister also used the opportunity to commend Kanu and his team-mates over what he described as the "impressive performance" of the Super Eagles against the Harambee Stars of Kenya, while thanking Nigerians for the massive support during the encounter.
In his response, Kanu expressed happiness over the honour done to him, noting that Nigerians have begun to recognise heroes in their lifetime. He further pledged total support for the Centre, expressing happiness over the honour.
He said: "I am happy and proud to be named Goodwill Ambassador of the International Cancer Centre, Abuja, and I want to assure you, sir, that I will use everything within my disposal to support the Centre.""

Cancer Incidence and Survival By Major Ethnic Group,England, 2002 - 2006

First Report on Cancer Incidence and Ethnicity

The National Cancer Intelligence Network (NCIN) and Cancer Research UK has today (Thursday) published the first report on cancer incidence and ethnicity.
According to the report, black people were nearly twice as likely as white people to get stomach cancer.

And black men were up to three times more likely than white men to get prostate cancer.

The report will help to shape policy on targeting relevant public health messages to the ethnic communities around the signs and symptoms of cancer.

It taps into sets of data from NHS Trusts and cancer registries that have never been linked up nationally - bringing together information that will be crucial for healthcare commissioners deciding how best to spend their budget in areas with large ethnic groups.

The report is the first national analysis of cancer incidence in ethnic groups and looked at all cases of cancer diagnosed in England between 2002 and 2006.

Professor David Forman, information lead for the NCIN, said: "While the white population is at a higher risk overall from cancer, this report highlights the increased risk of certain cancers, like stomach and prostate cancer, in the black population.

"We don't know why these differences exist. The reasons could mainly be genetic, but we think that lifestyle factors could have a role to play.

"We now need more research to understand why these differences exist and to begin to tackle these inequalities."

The report - presented at the NCIN annual conference in Birmingham today - also found that black people were nearly three times more likely to get myeloma - a bone marrow cancer.
And Asian women could be up to 80 per cent more likely than white women to get mouth cancer.
Dr Lesley Walker, director of cancer information at Cancer Research UK, said: "This report is a hugely important step forward in understanding how such a complex disease affects people from different ethnic groups.

"The next step is to think about how we can target health messages appropriately - making sure different communities are aware of the signs and symptoms of the cancers that are more likely to affect them."

Professor Mike Richards, national cancer director, said: "The NCIN was formed to help improve cancer services through good quality data.

"This report is a very significant move towards this aim. Based on what it shows, we will be able to measure the effect of any policies we now target to ethnic minority communities."

UK-The Office's Gervais will design Manbag for charity

Ricky Gervais of The Office and Extras fame is going to design a man purse -- but it's for a good cause.

The U.K.'s Prostrate Cancer Charity (part of the Charity Aids Foundation) asked television stars and other celebrities to design a man's handbag for The Manbag Company.

Each unique design will be auctioned off to fight prostate cancer.Of course, Gervais is a funny man and a cool, self-effacing celeb. But, it'll be interesting to see if even he can make something as inherently uncool as a man purse into swag a guy will want to spend money on -- cancer be damned.

Other celebrities (and pseudo-celebrities) joining Gervais in the bag designing business include Spandau Ballet, Depeche Mode, Alan Carr and Jade Jagger.Sure, it's all for a great cause and should be a huge success.

But, there will always be men out there who'd rather risk prostate cancer than be seen with a Manbag.

I don't recite many personal stories from the media but I think this is worth posting for many reasons.............

CANCER TAKES 'COURAGEOUS' PAPA


GARY PAPA, the WPVI-TV (Channel 6) sports director whose six-year battle with prostate cancer ended yesterday afternoon with his death at age 54, didn't just fight the good fight.

He fought it in public.

For the region, Papa became the face of someone living with cancer, not merely suffering from it. Showing up on camera without hair when he was undergoing treatment, Papa used his cancer as an impetus to educate others about the disease and to raise money for research.

"He's a profile of courage," Scott Palmer, a former Channel 6 colleague who now works as director of public affairs for the Phillies, said yesterday.

"To go through what he has gone through in the last couple of years, and more so the last couple of months, is extraordinary, but to do that publicly, it's incredible," Palmer said. "Through it all, he's been a model of courage and enthusiasm, which he's always had, but has continued to have through this time."

Papa, who last appeared on air May 13, is survived by his wife, Kathleen, and their two young sons, Nathaniel and Tucker. He had been at home as recently as last week, a Channel 6 spokeswoman said. He died at the Hospital of the University of Pennsylvania.

Channel 6 last night devoted its 5 p.m. newscast to "Remembering Gary," with a screen crawl devoted to viewers' reactions. "Gary Papa was amazing and it was obvious he loved what he did," one read.

Reporters were dispatched across the region to interview Papa's fans.

They included Gov. Rendell, who told anchors Rob Jennings and Monica Malpass: "I thought he was going to make it." Mayor Nutter told them, "I've never seen Gary Papa not smiling . . . so it was hard when I heard the news a little while ago."

Papa's chair was left empty throughout the newscast out of respect, Malpass noted.

The station urged viewers to join members of its staff at tomorrow's seventh annual "Run 4 Your Life" Father's Day Prostate Run/Walk to raise money for prostate-cancer research.
Papa was co-chairman of the event, which begins at 8:30 a.m. at the Philadelphia Museum of Art and proceeds along Martin Luther King Jr. Drive.

Papa's death was also the only story on "Action News" at 6 p.m. yesterday. Anchor Jim Gardner and weatherman Dave Roberts, at times blinking back tears, reminisced about their friend, who had worked at the station since 1981.

Papa, who had earned a degree in political science from Cornell University and a law degree from the University of Buffalo, was hired as a sports reporter and weekend sportscaster, having previously worked at stations in his native Buffalo and in Steubenville, Ohio.
He broke into TV sports at WGRZ-TV Channel 2, the same Buffalo TV station that produced both Gardner and Roberts.

From 1983 to 2008, Papa also hosted Channel 6's "Prime Time" magazine.
"Needless to say, all of us who have worked with Gary and who loved Gary are devastated tonight," Gardner said in a prepared statement before the newscast.

"Most of us knew this day would have to come eventually, but there's no way to really prepare," Gardner said. "There is an enormous hole in our hearts, almost as enormous as was Gary's spirit, his devotion to his work and to all of you."

John Clark, a sports anchor at NBC-10, said Papa was a longtime friend and a mentor to him.
"The night before my interview at Channel 10, I was coming up from the shore and didn't really have a place to stay and Gary said, 'Come over and sleep at my house.' So I stayed over his house," Clark said. "He was one of the most giving people I have ever met."

"Gary was a fighter; he gave it his all," said Comcast-Spectacor chairman Ed Snider in a prepared statement. "He was one of the good guys. We will always remember him for his passion for sports and his dedication to the sports scene in Philadelphia."

Eagles chairman and chief executive officer Jeffrey Lurie said in a prepared statement: "I will always remember him for the energy and passion he brought to his profession, especially during some of the toughest times of his battle with cancer."

In a Daily News interview published June 19, 1986 - exactly 23 years before his death - Papa told sportswriter Kevin Mulligan that he had begun knocking on doors in Buffalo for a TV job upon graduation from law school and he recalled how Channel 2 had hired him.

"I'd have taken anything," Papa said. "I said I was looking for a job in news, on the air, whatever. The guy said, 'Sorry, I've got nothing.' As I was on my way out, he said, 'By the way, our weekend sportscaster's leaving.' I told him I'd have a tape on his desk the next morning."
He was hired a week later.

Papa married Kathleen, a delivery-room nurse at Lankenau Hospital, in October 1985, two years after meeting her on a blind date arranged by "Action News" weekend anchor Jennings.
On their wedding day, Papa ran in a four-mile race and took sixth in the over-30 age group.
"My wife was going crazy," he told Mulligan. "I always wondered what I'd have done if I broke my leg or something. Better yet, what would my wife have done? On the trophy, it says, 'In defiance of matrimony.' "

Ireland-New Prostate Cancer Clinics

There are eight new Rapid Access Clinics being set-up in Ireland to fast track early detection and treatment of the disease.I hope they take on board the recent AUA Guidelines relating to initial testing or improve on them but just my opinion.

New prostate cancer clinic for Galway

A new Rapid Access Clinic to ensure early diagnosis of prostate cancer is to open in Galway.

The service is one of two schemes being piloted at St James's Hospital in Dublin and University Hospital Galway.

The National Cancer Control Programme says the service will be available nationally at all designated cancer clinics within 12 months.

Prostate cancer affects almost 2,500 men in Ireland on an annual basis. More than 500 die each year from the disease.

The highest death rates occur in the west of Ireland.

Currently patients are referred to urology clinics in over 20 hospitals nationwide, leading to widespread delays in receiving diagnosis.

The development of Rapid Access Clinics will fast track early detection and treatment of the disease.

Patients will have results within two weeks of referral, and if needed, will be provided with immediate access to specialist cancer care.

The National Cancer Control Programme plans to roll out the service in each of the eight national centres of excellence over the coming months.

Prostate Cancer Canada (amalgamated with....)

United Front in Prostate Cancer Fight

Prostate Cancer Canada has amalgamated with the former Canadian Prostate Cancer Network, now known as the Prostate Cancer Canada Network, which services 120 local support groups and 30,000 survivors. It is currently exploring a similar union with other prostate cancer organizations, says Rocco Fazzolari, Prostate Cancer Canada's vice-president of finance.

"There are several organizations across Canada dedicated to prostate cancer research, education and support" he says. "If these talks are successful, the result will be an organization with a national scope and regional presence."

The union of Prostate Cancer Canada and other significant groups would make enormous sense, Mr. Fazzolari says. It would eliminate competition for fundraising, form a solid national base to increase funds being raised, reduce fundraising and administration costs, avoid duplication and allow all stakeholders to speak with a single voice.

Urologists: PSA testing is case-by-case decision

Urology Times interviewed urologists about their opinions on routine PSA testing and whether these two studies have influenced how they will test for prostate cancer. For the most part, the urologists said they believe prostate cancer testing is a highly individualized decision between the physician and patient, and most don't anticipate that these studies will affect how they practice.

Report says 650,000 cancer deaths avoided between 1990 and 2005

The American Cancer Society's annual cancer statistics report finds that 15 years of steady declines in overall cancer death rates translate to about 650,000 deaths from cancer avoided or delayed during that time period. The estimate comes from 'Cancer Statistics 2009,' published early online and appearing in the July/August print issue of CA: A Cancer Journal for Clinicians.

The report says the overall cancer death rate in men decreased by 19.2 percent between 1990 and 2005, driven largely by decreases in lung, prostate, and colorectal cancers, while it decreased in women by 11.4 percent between 1991 and 2005, driven by decreases in breast and colorectal cancer. The number of avoided deaths increased in the later years as the gap between the rate in 1990-1991 and current mortality rates continued to grow.

The annual Cancer Statistics report and the 58th edition of the consumer-friendly companion report, Cancer Facts & Figures, also estimate that there will be 1,479,350 new cancer cases (766,130 in men and 713,220 in women) and 562,340 cancer deaths (292,540 among men and 269,800 among women) in 2009, corresponding to more than 1,500 cancer deaths per day. Lung, prostate, breast, and colon cancers continue to be the most common fatal cancers. These four cancers account for half of the total cancer deaths among men and women. Lung cancer surpassed breast cancer as the leading cause of cancer death in women in 1987, and is expected to account for 26 percent of all female cancer deaths in 2009.

Overall cancer incidence rates also decreased in the most recent time period in both men (1.8 percent per year from 2001 to 2005) and women (0.6 percent per year from 1998 to 2005). These drops in newly diagnosed cancers were largely due to decreases in the three major cancer sites in men - lung, prostate, and colon and rectum [colorectum, commonly referred to as colon cancer] - and in two major cancer sites in women - breast and colorectal cancers.
Among men in 2009, cancers of the prostate, lung, and colon will account for half of all newly diagnosed cancers. Prostate cancer alone will account for about 25 percent (192,280) of cancer cases in men. About nine in ten of these new cases of prostate cancer are expected to be diagnosed at local or regional stages, for which five-year relative survival approaches 100 percent.

The three most commonly diagnosed types of cancer among women in 2009 will be cancers of the breast, lung, and colon, accounting for 51 percent of cancer cases in women. Breast cancer alone is expected to account for 27 percent (192,370) of all new cancer cases among women.

Each year, Cancer Facts & Figures features a Special Section highlighting a particular topic related to cancer. The Special Section of Cancer Facts & Figures 2009 is 'Multiple Primary Cancers.' The purpose of this year's Special Section is to provide information for cancer survivors and health care providers about the burden and risks of subsequent primary cancers. It is estimated that there are now more than 11 million cancer survivors in the U.S., more than three times the number in 1970. Overall, cancer survivors are about 14 percent more likely to develop a new cancer than individuals who have never had a cancer diagnosis; almost 900,000 cancer survivors have been diagnosed with more than one cancer. Patients diagnosed with tobacco-related cancers, such as cancers of the oral cavity, lung, esophagus, kidney, and urinary bladder, have the highest risk for a second cancer because smoking is a risk factor for at least 15 types of cancer. Breast cancer survivors comprise almost half of women who develop a second cancer.

"A drop of one or two percent per year in the cancer mortality rate may sound small, but as this report shows, that adds up to 650,000 cancer deaths avoided over 15 years," said John R. Seffrin, Ph.D., American Cancer Society chief executive officer. "And because the rate continues to drop, it means that in recent years, about 100,000 people each year who would have died if cancer death rates had not declined are living to celebrate another birthday. That is undeniable evidence of the lifesaving progress that we as a country must dedicate ourselves to continuing."

'Cancer Statistics' and Cancer Facts & Figures have become critical tools for scientists, public health experts, and policymakers in assessing the current burden of cancer. The estimated numbers of cases and deaths in the current year are some of the most widely quoted cancer statistics in the world. The American Cancer Society's leading team of epidemiologic researchers, in collaboration with scientists from the National Center for Health Statistics (NCHS), compiles and analyzes incidence data from the National Cancer Institute, Centers for Disease Control and Prevention, and the North American Association of Central Cancer Registries, and mortality data from NCHS to estimate the number of new cancer cases and deaths for the current year nationwide and in individual states. Estimates of the expected numbers of new cancer cases and cancer deaths should be interpreted with caution, however. These estimates may vary considerably from year to year, particularly for less common cancers and in states with smaller populations. Despite these limitations, the American Cancer Society's estimates of the number of new cancer cases and deaths in the current year provide reasonably accurate estimates of the burden of new cancer cases and deaths in the United States. Such estimates will assist in continuing efforts to reduce the public health burden of cancer.
